Transitional Care Services:
Graham Regional Medical Center focuses on creating an individualized care plan based on your needs for transitional care. Out program is designed to improve your functional, medical, and overall wellbeing.

"Transitional Care" is when an acute care patient is ready to "transition" to less intense needs such as skilled nursing or rehabilitation. Our program provides several therapy options to care to your rehabilitation needs.

Graham Regional Medical Center offers private rooms, state-of-the-art technology, skilled physicians, therapists, specialists and daily meals catered to your prescribed diet.

Transitional Care Qualifications:

Transitional care or skilled nursing and rehabilitation must be related to an acute hospital stay.

A doctor must determine a need for daily skilled care such as physical therapy or skilled nursing.

A patient must be hospitalized as an "acute care inpatient" (not as an observation patient) for a minimum of 3 consecutive midnights within the last 30 days.

An admission can come from any facility after 3 midnights of inpatient acute care.
