Demonstrating Community Need
Graham Regional Medical Center (GRMC) conducts a Community Health Needs Assessment (CHNA) every three years. This assessment helps GRMC better understand the health needs of the people we serve and guides our efforts to improve the health and well-being of residents throughout our community.

In 2026, GRMC completed its latest Community Health Needs Assessment, which provides a comprehensive evaluation of the health status, demographics, and healthcare challenges facing Young County, Texas.

The assessment includes:

Analysis of the most current health and demographic data available

Input from community members, healthcare providers, local organizations, and community leaders

Identification of the most significant health needs affecting our community
The findings of the assessment are used to develop an Implementation Plan, which outlines specific strategies and initiatives GRMC will pursue to address priority health needs over the next three years.

The 2026 Community Health Needs Assessment and Implementation Plan will be presented to the Graham Regional Medical Center Board of Directors on October 22, 2026. Once adopted, these documents will serve as a roadmap for community health programs, partnerships, and services that support both the health of our residents and GRMC's mission.
